These Terms and Conditions constitute a binding agreement between you and Louisiana Children’s Medical Center and each of its subsidiaries and affiliates (together “LCMC”). By opening a LCMC Health Patient Portal account and using this service, you agree to be bound by these Terms and Conditions. By agreeing to these Terms and Conditions, you acknowledge that you are requesting access to portions of your personal health information and the ability to communicate with your LCMC health care team over the Internet using an electronic application called LCMC Health Patient Portal.

LCMC may modify these Terms and Conditions at any time, and you, as a user of the LCMC Health Patient Portal, agree that such modifications are effective immediately upon posting of the modified version on this website. LCMC may also limit or discontinue your use of the LCMC Health Patient Portal at any time for any reason.

LCMC Health Patient Portal is offered as a courtesy to LCMC’s patients. If you do not agree to be bound by these Terms and Conditions, please promptly exit this website.

LCMC is pleased to offer our patients online access to portions of their medical record through the LCMC Health Patient Portal ("website”). Patient access includes records created on the date LCMC Health Patient Portal service is activated (the “Activation Date”), as well as some records that may have been created before the Activation Date and records that are created after the Activation Date.

At this time, LCMC provides the following services through the LCMC Health Patient Portal:

  • Request or cancel an appointment
  • View and/or download selected portions of your medical record
  • Receive selected test results
  • Communicate with your health care provider via secure messaging regarding health concerns
  • Access (through links) to other sites which can provide additional health information

Minimum Technical Requirements and Security

  1. This website uses encryption software. In order to access LCMC Health Patient Portal, you must have:
  • A personal computer equipped with an Internet browser that is Secure Sockets Layer (SSL) compatible (such as the most current version of Internet Explorer, Chrome, and Firefox), or a terminal with 128-bit encryption enabled and configured to accept cookies.
  • Internet access
  • A valid email account to enable you to receive email messages. You may need to adjust spam filters to accept email from PatientPortal@LCMCHealth.org
  • Adobe Reader (To download a free copy of Adobe Reader, please go to) https://get.adobe.com/reader/
  • While LCMC uses state-of-the-art security, no system can guard against all risks of intentional intrusion or inadvertent disclosure of information. When using the LCMC Health Patient Portal, information will be transmitted over a medium that is beyond the control of LCMC or its contractors. BY USING THIS SERVICE, YOU HEREBY EXPRESSLY ASSUME THE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE. Once information is received by us, your medical information will be treated as confidential and given the same protection that all other LCMC medical records are given.
  • Setup and Use of LCMC Health Patient Portal

    1. LCMC Health Patient Portal has been designed to be a secure Internet-based environment through which you may receive confidential medical information about yourself. To enroll in LCMC Health Patient Portal, you will use your access code. The access code you receive will give you the opportunity to create a personal login name (called “LCMC Health Patient Portal Username”) and a password. Once this access code has been used or after 60 days of assignment, it will no longer be valid.
    2. Once your My Chart account has been activated, your LCMC Health Patient Portal account will become the primary vehicle to receive important messages from your participating provider(s) and other personal medical information. You MUST provide us with your email address when activating your LCMC Health Patient Portal account and keep your email address current using the Preferences option within LCMC Health Patient Portal to ensure that you receive notification of newly released information in a timely manner. You will continue to receive communications via the United States Postal Service or other means from LCMC.
    3. Proxy Access. You may request a LCMC Health Patient Portal account for yourself, for your minor children, and any other person who has given you written authority (a “Proxy”). You may not request or access a LCMC Health Patient Portal account for anyone else. By accepting these Terms and Conditions, you certify that you are authorized to open or access the account you are requesting and that all information you have provided to LCMC is true and correct to the best of your knowledge. You further agree that in the event you request Proxy access to a person’s LCMC Health Patient Portal account, you will be required to provide documentation of your authority (for example, a durable healthcare power of attorney) before you can receive an access code to complete registration or access that person’s health information. In certain circ*mstances, a minor (upon turning 13 years of age) may manage their own treatment and related medical records without a parent’s or guardian’s consent. Therefore, when creating a MyChart account for a minor, LCMC Health may take steps to verify the minor’s identity or confirm that the minor consents to their parent or guardian controlling their MyChart account. Once the patient turns 18, and is no longer a minor, any patient/guardian access will automatically be terminated.

    Privacy & Security: How We Use and Protect Your Information in LCMC Health Patient Portal

    1. Notice of Privacy Practices. LCMC considers the privacy of your health information to be one of the most important elements in our relationship with you that LCMC takes very seriously. Please review LCMC's Notice of Privacy Practices (NPP) for a thorough description of how we gather, use and protect your confidential medical information. By accessing or using the LCMC Health Patient Portal, you acknowledge that you have received a copy of your provider’s Notice of Health Information Privacy Practices, which may be accessed as follows:

    Copies of the NPP are also available at each LCMC facility.

  • All communications between you and health care providers using the LCMC Health Patient Portal occur over a secure connection. You will receive Internet email messages notifying you that new information is available in your LCMC Health Patient Portal account. These email messages will not contain any confidential medical information.
  • Your Responsibilities:

    1. YOU MUST NOT SEND ANY MESSAGES REQUIRING URGENT ATTENTION USING LCMC Health Patient Portal. Doing so may delay your receiving necessary medical treatment and could result in loss of life, organ or limb and could cause significant deterioration in your health. If you believe you have an urgent medical matter, you should call 911 immediately or go to the nearest emergency room. If the matter is not urgent but requires a timely response, please call your physician’s office.

      FOR ALL MEDICAL EMERGENCIES, CALL 911 IMMEDIATELY.
      Protect Your Password. As a LCMC Health Patient Portal user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis and 2) Keeping your login ID and password confidential. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will be able to read your messages and send new messages as if they were you. It is your responsibility to prevent the disclosure of your Login name/ID and password, and to change your password if you feel that your security has been compromised.You may change your password at any time using the Preferences link within the LCMC Health Patient Portal. If you lose or forget your username or password, you may retrieve it online via the Forgot Password link, by calling the toll-free number 866-662-6161 or email PatientPortal@lcmchealth.org

    2. Be aware that sensitive information that you may not want others to see may be contained in the LCMC Health Patient Portal. Information such as substance abuse, HIV status, cancer diagnoses, etc. may be posted in the LCMC Health Patient Portal. Please keep this in mind when you give your partner or family member access to your LCMC Health Patient Portal account. While this may not be of concern to you now, should you ever be tested for or diagnosed with a sensitive diagnosis, persons who have access to your LCMC Health Patient Portal account will be able to see this information. If you do not want anyone to see this information, you may want to restrict access to your LCMC Health Patient Portal from others. LCMC is not responsible for damages arising from a breach of confidential medical information resulting from sharing or mishandling your password.
    3. LCMC Health Patient Portal Messaging
      1. The LCMC Health Patient Portal Messaging is intended to facilitate dialogue regarding personal health matters. By using the LCMC Health Patient Portal Messaging, you agree not to use LCMC Health Patient Portal to post or send any illicit or offensive material.
      2. You understand that not all LCMC physicians participate in sending and receiving electronic communications. If one of your physicians is not listed as a potential recipient of email through the LCMC Health Patient Portal do not use the Patient Portal Messaging to attempt to contact him/her electronically. Please call to make an appointment or to speak with that physician.
      3. While the LCMC Health Patient Portal supports many different types of communication, it is best for you and your health care provider to agree on the type of communication most appropriate in your particular case. LCMC physicians/providers reserve the right to decline email correspondence with patients for any reason. If your request to correspond by email is declined, please call your healthcare provider to make an appointment or to speak with that physician.
      4. When you use the LCMC Health Patient Portal Messaging, your messages will be routed to the appropriate department as necessary for handling, and therefore LCMC employees, other than your healthcare provider (physician, nurse practitioner and/or physician’s assistant), may be involved in addressing your request. If your physician is out of the office or unavailable to respond, messages sent through the LCMC Health Patient Portal may be routed to other authorized healthcare providers within LCMC to facilitate a timely response to your request. Therefore, the LCMC Health Patient Portal may not be appropriate if there is sensitive information that you want to discuss directly with your physician.
      5. You understand that your LCMC health care team may send you messages, test results, and other communications through the LCMC Health Patient Portal as authorized in the sole discretion of your physician. These messages may contain information important to your health and medical care. It is your responsibility to monitor these messages. By entering your e-mail address at registration, you have enabled LCMC to notify you of messages sent to your LCMC Health Patient Portal Inbox. You further agree that LCMC will not be liable for any loss, injury, illness, damages, or claims of any kind resulting from your failure to timely read messages and take appropriate action when warranted based on the messages you may receive through the LCMC Health Patient Portal.

    Your Medical Record

    1. As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including the results of all tests, procedures and therapies. The LCMC Health Patient Portal is an internet application that enables a patient to have secured Web-based access to personal clinical information, as released by the treating physician, and allows secure electronic messaging with participating LCMC physicians. By using the LCMC Health Patient Portal, you understand that not all of your medical record will be accessible through your LCMC Health Patient Portal account. Posting of information to your LCMC Health Patient Portal account is at the sole discretion of your of LCMC and/or your healthcare provider.
    2. Messages you send or receive through the LCMC Health Patient Portal become part of your permanent medical record; they will be accessible to current and future providers who are involved with your care.

    Discontinuing Use of LCMC Health Patient Portal

    1. You may discontinue your use of the LCMC Health Patient Portal anytime by submitting an online request through customer service or calling the toll-free number 866-662-6161.
    2. If access is restricted for planned maintenance of the LCMC Health Patient Portal system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to your LCMC Health Patient Portal without advance notice.
    3. LCMC reserves the right, in its sole discretion, to limit or discontinue your use of the LCMC Health Patient Portal at any time and for any reason, including but not limited to your use of LCMC Health Patient Portal on behalf of another person. You will be notified if your LCMC Health Patient Portal service is discontinued.

    Links to Third Party Websites

    This website and the LCMC Health Patient Portal may contain links to websites operated by other parties which might have information on health topics of interest to you. THESE LINKS ARE PROVIDED FOR YOUR GENERAL INFORMATION AND EDUCATION ONLY AND, SHOULD NOT BE REPLIED ON FOR PERSONAL DIAGNOSIS OR TREATMENT. LCMC does not sponsor or endorse any of these sites, and makes no representations or warranties as to the accuracy of the information contained on these websites. LCMC has no control over the security or privacy practices of these external websites.     Limitation of Liability

    1. Some of the material on LCMC’s website and in the LCMC Health Patient Portal is provided by third parties and LCMC will not be held responsible for any such third-party content. LCMC disclaims any responsibility for or liability related to such third-party material. Any questions complaints or claims related to any product should be directed to the appropriate third party.
    2. LCMC will not be liable for any loss, injury, illness, damages, or claims of any kind resulting from your failure to timely read messages, comply with these Terms and Conditions, or take appropriate action when warranted based on the severity of your health condition or messages you may receive through the LCMC Health Patient Portal.

    Restrictions on Use and Ownership

    All content included in the LCMC Health Patient Portal and on all LCMC web sites, including, but not limited to, text, photographs, graphics, artwork, images, names, logos, trademarks, service marks and data ("Content"), in any form are protected by U.S. and international copyright law and conventions. The Content includes both Content owned or controlled by LCMC, its subsidiaries or affiliates (“LCMC”) as well as Content owned or controlled by third parties and licensed to LCMC. Reproduction of any Content, in whole or in part, by any means, is prohibited without the express written consent of Louisiana Children’s Medical Center.

    Governance

    This agreement shall be governed by and construed in accordance with the laws of the State of Louisiana.

    Definitions

    The terms hereby used in this agreement are used in their generally prevailing meaning unless defined hereafter.

    1. LCMC Health: includes but is not limited to Children’s Hospital, Children’s Hospital Medical Practice Corporation, Crescent City Physicians, Inc., New Orleans East Hospital, Touro Infirmary, University Medical Center, West Jefferson Medical Center, East Jefferson General Hospital, and the employees and members of the medical staffs of each of the foregoing.
    2. Healthcare Providers: includes but is not limited to physicians, residents, fellows, registered nurses, licensed practical nurses, physician assistants, nurse practitioners, nurse assistants, pharmacists, occupational therapists, physical therapists, speech therapists, audiologists, rehabilitation counselors, social workers, psychologists, dentists, dental hygienists, lab and radiology technologists, and others employed by or affiliated with LCMC Health.
    3. Guardian: is a person who has the legal authority and duty to care for another person especially due to the other person’s age, incapacity, or disability. A guardianship is an involuntary trust relationship in which one party, called a guardian, acts for an individual called the ward.
    4. Notarize: the act of officially certifying a legal document by a notary public. The purpose of having a legal document notarized is to ensure the authenticity of the signatures that appear on the document.
